网织红各项参数在临床贫血病人中的应用研究
【摘要】目的：研究诊断溶血性贫血、再生障碍性贫血与网织红细胞各项参数
指标的相关关系。方法：选取河北省医院近两年的病人，一共 80 位贫血病例，
此次论文的数据是以其中有溶血性贫血 30 例，再生障碍性贫血 50 例以此设立
为实验组,另外再选取 20 位健康的人设立为健康组为参考数据。网织红细胞各
项参数指标选用迈瑞 BC6900 全自动血液分析仪来分析:依据高荧光强度网织红

细胞百分比（HFR%）、中荧光强度网织红细胞百分比（MFR%）、低荧光强度网

织红细胞百分比（LFR%）、网织红细胞百分比（RET%）、网织红细胞绝对值

（RET#）和幼稚的网织红细胞比率（IFR%）的具体数据统计，对其进行分析并

检验结果。结果：通过贫血病人和健康人员的网织红细胞不同的参数指标的对

比，可以发现溶血性贫血病人的 RET#、RET%、IRF%、MFR%、HFR% 大幅度上升，

LFR% 大幅度下降（P<0.05）；再生障碍性贫血患者 RET#、RET%显著降低，IRF

%、MFR%、HFR%显著升高（P<0.05），而 LFR%轻度降低（P<0.05）。结论：网

织红细胞相关参数指标可以用来诊断不同类型的贫血病症,以及可以用来判断病

人贫血状况程度,进而能提供一些数据在临床上。

【Abstract】Objective: To study the relationship between the diagnosis of hemolytic

anemia, aplastic anemia and reticulocyte parameters. Methods: A total of 80 cases of

anemia were selected from patients in Hebei Province in the past two years. The data

of this paper were 30 cases of hemolytic anemia and 50 cases of aplastic anemia. The

experiment was set up as an experimental group, and another 20 were selected. A

healthy person is set up as a reference data for the health group. The parameters of

reticulocyte were selected by Mindray BC6900 automatic blood analyzer: percentage

of reticulocyte with high fluorescence intensity (HFR%), percentage of medium

fluorescence intensity reticulocyte (MFR%), percentage of low fluorescence intensity

reticulocyte Specific data statistics (LFR%), reticulocyte percentage (RET%),

reticulocyte absolute value (RET#), and naive reticulocyte ratio (IFR%) were

analyzed and tested. RESULTS: By comparing the different parameters of

reticulocytes between anemia patients and healthy people, it can be found that RET#,

RET%, IRF%, MFR%, and HFR% of patients with hemolytic anemia increased

significantly, and LFR% decreased significantly ( P<0.05); RET# and RET% were

significantly decreased in patients with aplastic anemia, IRF%, MFR%, and HFR%

were significantly increased (P<0.05), while LFR% was slightly decreased (P<0.05).

Conclusion: Reticulocyte-related parameters can be used to diagnose different types

of anemia, and can be used to determine the degree of anemia in patients, and thus

provide some data in the clinic.

前言

由调查数据显示，全世界有将近 30 亿人患有各类不同程度的贫血症状，其

中有近数千万的人死于由贫血引起的疾病。就当下而言西方国家贫血病得病概

率远低于我国，调查显示，患病者中女性、老人和儿童属于高发人群，明显高

于成年男性。贫血是临床多见的病症，临床上将人体外周血红细胞容量低于正

常范围的下线称为贫血(anaemia)。在临床上诊断是否患有贫血症状和贫血程度，

是将病人的网织红细胞的各项参数指标与正常人的相比较来判断的，这是由于

外周血红细胞的总量难以测量，要求技术很高，不容易测出。我国诊断贫血的

指标标准稍低于国外，成年男性和成年女性血红蛋白浓度分别低于

120g/L、110g/L 当成判断标准，与成年人相比，小孩子的血红蛋白浓度大概低
